Apply for a Crusade Kentucky license plate today!

Supporters of the WHAS Crusade for Children can now show their loyalty on their cars and trucks.

The Kentucky Division of Motor Vehicle Licensing has approved this design for the WHAS Crusade for Children’s first ever specialty license plate.

The Crusade for Children needs 900 applications before the Division of Motor Vechicle Licensing can begin issuing the Crusade license plates. Tell all your friends!

• 1. Fill out the PDF APPLICATION FORM below, print it out, attach payment (make checks payable to WHAS Crusade for Children) and mail to:
• WHAS Crusade for Children
  520 W. Chestnut St.
  Louisville, KY 40202
• OR
• 2. Fill out the PDF APPLICATION FORM ONLINE and email it to deandra@whascrusade.org
• 3. The fee is $25.
• 4. We must reach a minimum of 900 applications by April 14, 2016.
